Update the list of supported platforms, how to use the tool, and rename
README to README.rst so that github does some nice formatting.

This tool isn't using $(TARGET) anymore to choose the build platform.
The build configuration is entirely determined by the U-Boot
configuration. Now to use the tool, run "make <target>_defconfig" where
<target> is the name of a file in the u-boot/configs directory.

README was missing some information about how to get the working
directory into a usable state, and patches that need to be applied to
the project.
