iommu/iova: Header cleanup

With all the various IOVA caching now driven only by core IOMMU
code, it makes little sense for related functions to be referenced
by generic code without CONFIG_IOMMU_IOVA selected. Clear out some
stubs and declarations which should no longer be needed.
