docs: Add missing running_apps_on_avh.md to mkdocs.yml

Also remove the copyright header from running_apps_on_avh.md, because
our guidelines do not require markdown files to have copyright headers,
and the copyright header prevents MkDocs from automatically detecting
and setting a page's navigation title.
